I am grateful to have been afforded the opportunity to provide counseling services to my clients over the past 11 years. I have decided to close my counseling practice and move on to new ventures in research and consulting.
Your assigned clinician will continue to be available to you for essential clinical services for a period of 45 days, therefore, as of May 31, 2021, you should consider your treatment over and that your clinician will not, in any continuing sense, be providing you with therapeutic services.
Additionally, we will send copies of your records to any future treatment providers if you authorize this. Records requests can be sent to [email protected].
Pursuant to law, my practice will maintain records for a period of seven years commencing on the date of termination of services or on the date of the last contact with the client, whichever is later. When the client is a child, the records will be retained for a period of seven years commencing either upon the last day of treatment or when the child reaches eighteen years of age, whichever comes later, but in no event, shall records be kept for more than twelve years.
